Dear Doctor
I have the below said problems.
1. Pain in the right ankle
2. Pain in the right sole, when I stand for few hours or several minutes.
3. Burning sensation in both the feet ( sole)
4. Numbness started in left foot If I sit for long ie. 2 to 3 hours
5. Burning in the foreskin.

I have undergone following tests. I have family diabetic history.
1. GTT – the results are normal except for 2 hrs result (after 2 hrs it was 137 Normal range is up to 130)
2. Once PP blood sugar was 173 as I have taken more sugar for breakfast.
3. Otherwise my sugar is just between 90 and 110. I have tested it many times.
4. Nerve conduction study (NCS), Neurologist ruled out the problems with nerve but prescribed some medicines for depression ( Serta, Olapin, and zolfresh). I am on these medicines for the past 15 days but for no improvement. Neurologist suggested if there is no improvement I may have to undergo CT scanning of spinal card.
5. Nero Psycologist suggested just to take the above medicines for 2 months and asked me not to undergo any tests / or consultation for 2 months.
6. I hv also taken mecobalamin tablets for a month and no improvement.
7. I hv also taken 3 \4 ME12 injection (500mcg) but for no improvement.
8. I hv also taken 10 – 15 Roff tablets but for no improvement.

I feel that these problems are due to erratic change in blood sugar level.

Pain in your padham is always due to some strain or sprain, caused by injury or undue exertion or occassionally due to oncoming Arthritis. The leading cause, you already know now, or see an Orthopaedist who has the know-how.

THE SOLE REASON FOR YOUR PAIN IN YOUR SOLE NEED NOT BE DIABETES ALONE. This may have gone wrong with some injury sustained while landing from a step or a vehicle or ocassionally due to a calcanial spur (a small spiky growth in the heel bone, named calcanium), which may need a local anaesthetic treatment. Generally, Diabetics have painless joint diseases,even fractures called charcots arthropathy. Only certain neural involvements cause severe pain. Methycobal injection is not a panacea for all neuropathic problems, foot or elsewhere. Your undulating blood sugar levels indicate more a impaired Glucose intolerance rather than a frank Diabetes.

Query :
I have had hypertension for the last 15 years. i have been taking the recommended drugs for its treatment. however, i find that the use of the drug, alsartan am, disturbs my sleep. what is the antidote to it. I have also been an athlete. I jog or do treadmill for half an hour everyday, followed by some weight training and yoga. all of this does not seem to help reduce my weight which is static at 90kg for the last 15 years.

Of late I also find that when I visit high altitude area I get mild headache and a rise in BP - which was not the case,say, five years ago. At that time I could walk for 5-6 hours at 12,000-14,000 ft without any trouble. In the given situation do you recommend some vitamins and mineral supplements or change of life style - I smoke pipe five to six times a day and get drunk at least twice a week.

May be true in your case. You have to rule out other causes of sleeplessness before changing your medication. At 50 years you have already enjoyed your full quota of smoking for this birth.
You have already smoked your last cigarette.

Your lack of sleeping may be due to midlife blues. Which is common for many a people who attain this age. Your responsibilities as a father to satisfy your grown up children’s demands and needs. As a husband trying to cope with wife needs. As a male you may not rise to the occasion, making vex at sex . As a responsible person you may have to meet many a person with varied situations leading to tension day in and out. Apart from all these your age itself invites ill-health and unmask the distained diseases programmed by the nature.

Some of the above or all of the above or sum of all may cause you sleep disturbances certain pluses and minuses in your life calculations will salvage you from the hopeless situation. You need zestful days and restful nights.

Your feeling of discomfort in the chest may be caused by your chimnified ( carbon deposited ) lungs failing to expand due to loss of elasticity, heart getting ballooned out due to on set of hypertension or gaseous abdomen again caused by your inveterate smoking addiction and accompanying acidity and overwhelming anxiety.

  • Quit smoking.
  • Have a physical (examination with a doctor)
  • Don’t be emotional.

Kindly clarify whether we can use ajinomoto (mono sodium gutamate) regularly. There are reports that it is bad for health.

Ajinomoto is an additive used to increase the flavor of many a chinese dish and to make them fluffy too. It can cause a sensastion of choking in the chest---mimicking an anginal or heart attack -- causing tremendous anxiety and may put you in ICCU too, shaving all your life's savings before saving your life. This is called "Chinese Restaurant Syndrome".

MY WIFE IS A PATIENT OF BRONCHIAL ASTHMA. SHE IS 50 YEARS OLD AND TAKING SALMETER INHALER TWICE A DAY FOR THE LAST 7 YEARS. WILL CONTINUOUS USE OF THIS MEDICINE HAVE LONG TERM EFFECT ON HER HEALTH? IS THERE ANY ALTERNATIVE?
